When you need to collect information, you can create a form in Microsoft Forms.
1. Login to office.com using your email address and password (similar to your laptop password)
2. On the left-hand menu, select Forms
3. Click arrow beside New Quiz and select New Form
4. Name your form and give it a description
5. Select Add new
6. Choose the type of question you want to add, then format the options for the question type you've selected.
7. Select Add New to continue adding questions from the variety of available question types until your form is complete.
8. If you want to see how your form will look, select Preview.
9. You can view how your form will look on a computer or on a mobile device.
10. To change the look of your form, select Theme, choose a different theme and preview your form again.
